“Negotiation is a critical skill for physicians, yet it is often overlooked in medical training. Whether discussing a starting salary, a promotion raise, or adjustments to workload and benefits, effective negotiation can significantly impact a physician’s career earnings and job satisfaction. In a world where health systems have dedicated teams of lawyers and compensation analysts, physicians negotiating alone are at a structural disadvantage — unless they come prepared.”
Chapter 11 covers the full arc of negotiation: from understanding your BATNA and the employer’s internal constraints, to preparing a documented case with benchmark data, to navigating the counter-offer and the periods immediately after signing. It includes worked examples, common red-flag clauses, and a pre-negotiation checklist physicians can use for any contract discussion.

Andrew Duncan is Chief Operating Officer of the Department of Orthopedic Surgery at the University of Pennsylvania Health System and a doctoral candidate in business at Florida Atlantic University, where his dissertation examines physician compensation methodology using CMS Medicare data.
His work at Penn Medicine gives him a practitioner’s view of how compensation structures are designed and implemented at one of the country’s largest academic health systems. This book bridges that operational experience with rigorous analytical methodology — written for the physician who wants to understand, not just accept, the economics of their work.
